Application privacy
The embedded Guest Application is an admissions and placement-screening form. Complete the screening questions the form requires. Depending on the application, those questions may include substance of choice, recent-use information, prior treatment program names, treatment dates and duration, current-program details, criminal-history categories, probation, and parole status.
Provide the requested answers in the fields provided. Do not upload or paste full medical records, medication lists, diagnostic reports, laboratory results, full treatment records, court documents, or other record files unless AFV gives you an approved secure method for sending them.
